Dr Middleton was born in Salt Lake City, Utah in 1933. Dr Middleton received his B.A. from the University of Utah in 1955 and his M.D. from Cornell University in 1958. Dr Middleton returned to Utah in 1967 to practice medicine with his father, Dr Richard P. Middleton, joining the medical school faculty on a half time basis. Dr Middleton was named Chair of the Division of Urology in 1968, serving in that capacity until 1991. Dr Middleton is Professor Emeritus of Surgery. |
