Insertion of a central line (CL) for surgery is one to be carefully considered. This case study looks at the decision to insert a CL in a patient with Cerebral Palsy (CP) at the end of surgery due to unexpected intraoperative changes. There are cases in which insertion of a CL is an absolute and those where it is not. There also are many cases in which a CL would be useful yet was not chosen in the initial anesthetic plan. Current research was reviewed about the most appropriate insertion techniques by student registered nurse anesthetists (SRNA), which concluded using ultrasound (US). The review of line size and delivery rates are also covered to help in the decision of what line will be needed. Indications for line placement are discussed. During any period of care in which placement of a CL is needed, the goal of its insertion will help dictate the type of catheter chosen and the most appropriate location, type and size to be utilized.
